Band syllabus for Fall 2010 semester. This document should be signed and returned by August 13, 2010.
This form must be signed and returned for all students participating with the band.
This form must be filled out each individual time the band travels during school hours.
This form must be filled out by all student traveling out of the county. This form must be signed by the parent, doctor (if medication is prescribed) and must be notarized.
All students must sign this form in order to participate with the band.
